In the Season 4 premiere, Michael finds Sara alive, held captive. She explains: Gretchen showed her a dead woman who looked like her, then faked Sara's execution to break Michael psychologically. Why? To control him. The "head" was never actually Sara's.
The decision backfired spectacularly. The fan base went "collectively ape scat". Viewers felt "shocked" and "deceived" by the violent nature of the death. The outrage was not just that a beloved character had died, but that she had been killed off-screen in a particularly gruesome fashion—decapitation—over what appeared to be a petty contract tiff.
